Quinidine-induced Syncope.

چکیده
Since quinidine was introduced by Frey in 1918 it has remained the drug of choice for the conversion of certain arrhythmias to sinus rhythm and for the maintenance of normal rhythm once achieved. It is commonly used in this manner for atrial fibrillation, atrial flutter, -paroxysmal tachycardia of atrial, nodal, and ventricular origin, and ventriculat fibrillation. It has also been employed as a prevesnive of extrasystoles, particularly following recent myocardial infarction. Only in the past two years has its position in the management of these arrhythmias been challenged by the development of direct-current shock therapy (Lown, Amarasingham, and Neuman, 1962).

QUINIDINE occupies an almost unique place in the therapy of cardiac arrhythmias. In the 45-odd years of its clinical use only very few drugs possessing similar pharmacologic properties have been found, but none could match quinidine in its broad therapeutic applications. Consequently, the knowledge concerning its clinical use and toxicity is of considerable importance, for patients who tolerate...

S udden death resulting from medications prescribed with good intentions is an everpresent threat first recognized nearly a century ago (1,2). As early as 1923, when quinidine was first used as antiarrhythmic therapy, a disturbing phenomenon was noted: some patients treated with quinidine suffered from sudden collapses, sometimes ending in unexpected deaths (2). These events were first attribut...
متن کاملEfficacy of quinidine in high-risk patients with Brugada syndrome.
BACKGROUND Automatic implantable cardioverter-defibrillator therapy is considered the only effective treatment for high-risk patients with Brugada syndrome. Quinidine depresses I(to) current, which may play an important role in the arrhythmogenesis of this disease. METHODS AND RESULTS The effects of quinidine bisulfate (mean dose, 1483+/-240 mg) on the prevention of inducible and spontaneous ...
متن کاملThe Comprehensive In Vitro Proarrhythmia Assay (CiPA) Guide: A New Approach to Cardiac Risk Assessment
Historic Overview – Delayed Repolarization Hazard The association of certain cardiovascular drugs such as quinidine with risk of sudden cardiac death (quinidine syncope) was recognized nearly 100 years ago, but the mechanism was obscure until the 1960s, when quinidine syncope was found to be associated with prolongation of the electrocardiographic QT interval and a cardiac arrhythmia that displ...
متن کاملThe effect of acute administration of quinidine, dextromethorphan and combination of dextromethorphan/quinidine on pentylenetetrazole-induced clonic and tonic seizure thresholds in mice
Background: Dextromethorphan (DM) as a non-opioid anti-cough has neuroprotective effects. Combination of DM with quinidine decreases rapid metabolism of DM to dextrorphan (DX). This study aimed to examine the effects of acute administration of quinidine, DM and combination of dextromethorphan/quinidine (DM/Q) on pentylenetetrazole (PTZ)-induced clonic and tonic seizure thresholds in mice. Mater...
